especially since the nominal size is smaller than the actual physical size.

Pipe thread is a tapered thread-the top of the hole is larger but tapers smaller as it goes deeper. This ensures sealing. I think it doesn't work if the threads go completely through the hole.
Happened to me on some of my stuff and them pretty Fore Performance fuel rails had an issue like that.
